1.) First Step is to fill out your information for each camper. You can submit 3 ways.

             Download form and mail to the address on the form.

               Pick up a form at business locations.
                       Forms have been dropped at Maxwell locations of Logsdons, Toot's Cuttin                           Parlor, Main Street Floral, and Muscular Rehab Clinic. Collins locations                             are the Exchange State Bank. Library, and Hair Hut.

              You can fill out the form below and click submit.
                      You must read the flier and the waiver. When you fill out the form online,                          you are consenting to the waiver. Read waiver at the bottom of the form.

2.) Second Step is your payment.
To pay, you can send a check with the paper form to the address on the form or use this Paypal check out and pay in full.
